The Position
As a Home Care Physiotherapist, you’ll provide tailored, one-on-one care in clients’ homes across the Hobart region. You’ll manage a diverse caseload that may include NDIS, DVA, and Home Care Package clients.
Leap Health offer flexibility in spades—whether you’re after a casual role or part-time hours, they’ll work with your schedule. All work-related tasks like programming, reporting, and meetings are included in your paid hours.
Your responsibilities will include:
- Provide physiotherapy services to clients in their homes, ensuring person-centred, goal-oriented care
- Conduct assessments, develop treatment plans, and deliver rehabilitation tailored to each client’s needs
- Work closely with families, carers, and other health professionals to ensure optimal client outcomes
- Contribute to a positive team culture that supports ongoing learning and peer support
What we’re looking for:
- Tertiary qualification in Physiotherapy
- Current full AHPRA registration (or working towards)
- Current Australian driver’s license
- CPR & First Aid certificate
- A passion for community health and working one-on-one with clients in their own environment
- Strong communication skills and a collaborative mindset
- Current driver’s licence and access to a reliable vehicle
- Experience in aged care, rehabilitation, or community-based services is highly valued
- Ability to maintain complete and accurate clinical documentation
