The Position

As a Home Care Physiotherapist, you’ll provide tailored, one-on-one care in clients’ homes across the Hobart region. You’ll manage a diverse caseload that may include NDIS, DVA, and Home Care Package clients.

Leap Health offer flexibility in spades—whether you’re after a casual role or part-time hours, they’ll work with your schedule. All work-related tasks like programming, reporting, and meetings are included in your paid hours.

Your responsibilities will include:

• Provide physiotherapy services to clients in their homes, ensuring person-centred, goal-oriented care
• Conduct assessments, develop treatment plans, and deliver rehabilitation tailored to each client’s needs
• Work closely with families, carers, and other health professionals to ensure optimal client outcomes
• Contribute to a positive team culture that supports ongoing learning and peer support

What we’re looking for:

• Tertiary qualification in Physiotherapy
• Current full AHPRA registration (or working towards)
• Current Australian driver’s license
• CPR & First Aid certificate

• A passion for community health and working one-on-one with clients in their own environment
• Strong communication skills and a collaborative mindset
• Current driver’s licence and access to a reliable vehicle
• Experience in aged care, rehabilitation, or community-based services is highly valued
• Ability to maintain complete and accurate clinical documentation
